There are days when I'm totally fine and I get greeted with "are you okay? you look tired."
I'm not quite sure how to react to that. I thank them for their concern and assure them I'm fine. But then I start to feel insecure about the way I look. Do I look like shit or something? I really hate that comment, because if I feel fine and I think I look fine, then this is as good as I get, for the day. And for the rest of the day, I'm basically going to look tired.

Yeah, it's the "you look tired" that gets me too. I just got it from a colleague one night last week after a full workday of this-n-that. I mean, it was a tiring day, but my energy level was still decent. Maybe it's time to boost the eyecream dose.
